The handwritten title page reads: An Index Catalogue of American Agarics & Boleti; Being a list of such species as have been found in the United States, with references to catalogues and descriptions, compiled by Edward J. Forster, Boston, 1887.

The preface page notes: It is confidentiality believed the following bibliography contains [indecipherable] references to Agarics and Boleti found to date (1887) growing in the United States. Names of species inclosed with brackets are either synonyms or obsolete. Preceding the alphabetical index of Agarics, the species of this genus and of Cortinarius will be found arranged by subgenera. Following the bibliography an attempt has been made to arrange the references chronologically.

Biographical note

Dr. Edward Jacob Forster was born in Charlestown, Massachusetts on July 9, 1846, to Jacob and Louisa Webb Forster. He graduated from Harvard Medical School in 1868 and continued his studies in Paris and at the Rotunda Hospital in Dublin. On September 5, 1871, he married Anita Damon Lyon, with whom he had 3 daughters. He enlisted in active military service with the Massachusetts Volunteer Militia in 1864 and in 1895 he was appointed Surgeon General of Massachusetts. He was a member of several medical societies and served as treasurer of the Massachusetts Medical Society and was a member of the Massachusetts Horticultural Society. He died suddenly due to a stroke while aboard the steamboat Puritan traveling from New York to Boston on May 15, 1896.
